Who we are:
With over 20 years of experience providing rehabilitation services in post-acute care, our team identified a need for a comprehensive therapy program for patients in senior living communities. Our mission to deliver continuity of services and support CMS' value-driven model allows us to implement a full-circle approach to therapy. We work with the appropriate partners in assisted living communities, independent living communities and home health agencies to execute a comprehensive medical model that meets the needs of patients specific to their setting.

What we do:
Element provides functional, evidence-based rehab services to individuals living in AL, IL and their home. Our clinical services are relevant to the environment where our patients live, and goals are established in collaboration with the patients and families to ensure all concerns are addressed. We guarantee the highest quality of care for our patients while remaining fiscally vigilant.

Why we do it:
Post-acute care is typically the safest and most effective when delivered in a familiar environment, such as the patient's home. Our vision is to bring together all care providers to meet the needs of our patients and residents to improve their wellbeing and 'support a life well lived.'
What We Provide
Element is an approved in-network provider with Medicare Part B and other payers to provide the below therapy services in the patient’s home.
